2020 Guam Benefits

Attention: All Open Enrollment elections will be completed in Workday. If you are adding a dependent(s) to your coverage (spouse/domestic partner and/or child(ren)), you must also complete a Calvo’s Select Care Enrollment Form.

This is an “Active Enrollment” for Voluntary Life Insurance and LTD which requires you to make these elections within Workday. Your 2019 elections for these offerings will NOT carry over like in previous years.

Open Enrollment begins November 1st through 14th - this is when the elections will be live in Workday and you can take action.

  • Review the Guam highlight sheet.

  • Attend an informational session on October 25 (9:30AM - 10:30AM GMT) .

  • Enroll and/or make changes to your benefit selections in Workday starting November 1 to November 14.

  • Submit your finalized benefits selections on or before November 14 in Workday (no changes allowed after November 14).

  • Print and review your Workday confirmation page reflecting your selected benefits.

Medical, Dental, and Vision

Finish Line offers medical, dental and vision benefits through Calvo’s Select Care. If you are adding a dependent(s) to your coverage (spouse/domestic partner and/or child(ren)), you must also complete a Calvo’s Select Care Enrollment Form.


Annual Physical Incentive - Employees enrolled in the medical plan will receive a $100 cash incentive for getting their annual physical and will have the opportunity to earn another $100 for meeting 3 out of 5 health targets. For more details, review the Guam Benefits Guide. Additionally, spouses and domestic partners on the medical plan are now eligible to get the same incentive.

Paycheck and Life Insurance

Short Term Disability

Short Term Disability (STD)

Finish Line is excited to continue its investment in YOU in 2020 by covering the cost of Short Term Disability. Short Term Disability Insurance is "paycheck insurance" for when life throws unexpected events into your path.

STD protects an employee from loss of income in the event you would need time off of work due to a disability (illness, injury or pregnancy) for a short period of time. All full-time, regular employees will have STD at no cost beginning January 1, 2020 which will pay you 60% of your gross income for up to 13 weeks (1 week waiting period, 12 weeks paid). This benefit is 100% paid for by JD Finish Line.


Long Term Disability

Long Term Disability (LTD)

Paycheck Insurance - LTD protects an employee from loss of income in the event you would need time off of work due to a disability (illness or injury) for a long period of time. All full-time employees will have the option to purchase LTD which will pay you 60% of your gross income until you are able to return to work or reach 65 years of age.

JD Finish Line will pay 50% of the premium for employees who voluntarily elect Long Term Disability LTD.

Basic and Supplemental Life Insurance

Basic Life Insurance with AD&D

    • Company provided benefit at no cost to you.

    • All regular full-time store employees (FLM Store Manager and FLM Assistant Store Manager) employees have the benefit of one times base pay up to $100,000.

Supplemental Life Insurance

    • Offered to all full-time employees (FLM Store Manager and FLM Assistant Store Manager). If you want to sign up for, or increase your current coverage, we will be offering a one-time life insurance enrollment period for current employees

    • The Guaranteed Issue Amount is currently $180,000 for employees and $30,000 for spouse. You can enroll up to the Guaranteed Issue Amount without underwriting.

    • The minimum/maximum amount for dependent life insurance is $10,000.

    • You must enroll in supplemental life if you want to elect coverage for your spouse/domestic partner and/or child(ren).

Wellness Incentive Program


  • Earn cash for being healthy - Through our Wellness Incentive Program, you have the opportunity to earn Pulse Cash as you achieve each level. If you are on the medical plan, you can earn up to $150 Pulse Cash (which is redeemable for gift cards or can be deposited directly into your personal bank account) and a bonus $50 in premium discount every quarter.

  • Download the Virgin Pulse app today to begin earning points for tracking healthy habits, logging your activity into the exercise log, and participate in company challenges. New for 2020, Virgin Pulse is now available to all spouses/domestic partners on the medical plan. They are able to earn a $25 gift card every quarter.

  • Did you know that most pedometers are compatible with Virgin Pulse including Garmin, Apple Watch, Fitbit or even your phone?
